1. Download the following forms: Premier - Yankee Single Facility Affiliate Agreement
2. Complete the forms and FAX back Yankee Alliance, LLC. Alliance Member Support at 978-681-6100
3. As soon as we receive your completed membership forms we will send you a welcome packet with contract information.
4. You may create a user name and password for this website here. You will receive a confirmation email indicating your membership has been authorized. You will then be able to access detailed contract information on this website.

Physicians
1. To participate in our national program with Premier, Provider Select, MD please complete the Letter of Participation and FAX back to Patty Pennelli at 978-681-6100. In order to participate in Provider Select, MD you must purchase 80% of your medical/surgical supplies through McKesson General Medical.
